Under the Make America Healthy Again: Enhancing Lifestyle and Evaluating Value-based Approaches Through Evidence (MAHA ELEVATE) model, CMS plans to make $100 million available to support up to 30 proposals focused on health promotion and preventive care under three-year agreements.
CMS said the model offers Medicare beneficiaries additional care options without replacing existing medical services. The goal is to empower people with Medicare to lead healthier lives by addressing nutrition, physical activity, sleep, and stress.

The initial notice of funding opportunity will be released in early 2026, with a full program launch planned for September 2026. Insights from MAHA ELEVATE will help inform future Medicare coverage decisions and shape new payment models.
